I was wondering if there was a way to change that header text, (beginning with the subject of this post), that appears in the email to me when the person submits the form.

More specifically I'd like to change the date format to: December 31, 2003 at 12:07:06.

I would also like the url of the page they were using when they submitted it, since I will be using this form on multiple pages.

One additional thing, I have a "comments" field and user comments can get long where they're putting line breaks in. In the email that we receive with their information, any line breaks come across as <br>. Can it be formatted to just treat line breaks like they would normally?

Curently there is only on format of date in the script. Maybe in another version update I will give you the ability to choose the date format.

As for specifing what page it was sent from, I would just add a hidden field:

<input type="hidden" name="from_page" value="home">

And just change the value depending on what page the form is on.

Yeah, there are actualy several reasons for that, I actualy changed that feature around a couple of time with the <br> not breaking like so. I think I may also change that in a future update.
