PHP installation.

Form works well except that I can not get the personalized error page to fully work. I have a required email field that if I leave blank, correctly opens the error page with the email field highlighted.

I have a textarea field that correctly contains the comments submitted on the original form. That is, the submitted comments in the text area field show up in the textarea field on the error page.

I also have a radio field that I can not get to show up correctly in the error page. That is, the selected radio button is not selected on the error page.

Here is my radio button html on the error page:
<INPUT TYPE="RADIO" NAME="find" VALUE="1"<%RADIO%find {1}%>> 1
<INPUT TYPE="RADIO" NAME="find" VALUE="2"<%RADIO%find {2}%>> 2
<INPUT TYPE="RADIO" NAME="find" VALUE="3"<%RADIO%find {3}%>> 3
<INPUT TYPE="RADIO" NAME="find" VALUE="4"<%RADIO%find {4}%>> 4
<INPUT TYPE="RADIO" NAME="find" VALUE="5"<%RADIO%find {5}%>> 5

Here is my radio button html on the form page:
<INPUT TYPE=RADIO NAME="find" VALUE="1"> 1
<INPUT TYPE=RADIO NAME="find" VALUE="2"> 2
<INPUT TYPE=RADIO NAME="find" VALUE="3"> 3
<INPUT TYPE=RADIO NAME="find" VALUE="4"> 4
<INPUT TYPE=RADIO NAME="find" VALUE="5"> 5

Any help to determine why the selected radio button does not show in the error page would be appreciated. I likely forgot something obvious.

If I include the required email in the original submittal, all data including radio button selection is correctly emailed and the data file contains all correct information including the selected radio button info.

Where is your HTML form page located at (URL)?

When I go to the page:

http://dfclab.com/cta/sitefeedback/form.html

Which is the page that should be "personalized", none of the code in this post is contained in it. Edit that HTML page, IF THAT IS THE ONE YOU ARE CALLING, and then you will be all set.

Here is the error form:
http://dfclab.com/cta/sitefeedback/feedbackerror.html

Here is the form:
http://dfclab.com/cta/sitefeedback/form.html

It all works except for the radio buttons not showing which is selected on the page feedbackerror.html

You must access the form.html page from here http://dfclab.com/cta/index.php?page=feedback which is by design.
