  Subject: can the script be made to create the .dat file on its own?

I like that your script is easy to interpret, well done.

So far it seems that in order to have it write the results to a .dat file that file must already be created on the server. Can the script be made to autocreate the .dat file if one is not already present? The goal is so that the form creator (who knows nothing about .dat files, etc) can just create his forms and this script will automatically dump the HTML results into the .dat file. Emailing is not required, we're only interested in capturing the data to the .dat file. Is this possible?

Yes, if you set the script to write to a datafile that does not exist on your server, your server should create it. If it does not, it is a server configuration problem...
