We are on a windows server, and our host says they use blat.exe as their mail program. Does bizmail.cgi work with blat.exe and if so how should I populate the variables $uselib & $mailprog?

Ask your host this question. What is the path used for sending mail in form mail type scripts. When you get that post it back, and I will show you how to set it up. Seth
